Dr. M.A. Emick and Associates is pleased to be able to offer the Patient Portal service to you. Through the Patient Portal, you will be able to request appointments, submit forms, send non-emergency messages, and ask routine questions.

PLEASE NOTE: The Patient Portal is a convenience service; it is not intended to be a substitute for direct care.

In order to access the Patient Portal, you must agree to the following Terms and Conditions of Use:

  1. I understand that, by accessing or using the Patient Portal, I agree to be bound by these Terms and Conditions of Use.

  2. I understand that Dr. M.A. Emick and Associates may make revisions, changes, updates and/or improvements in products and/or services provided on the Patient Portal, or add new features, at any time, without notice.

    • I agree that such changes are effective immediately upon posting of the changed version on the Patient Portal.

    • I agree that any revised Terms and Conditions of Use will apply both to information Dr. M.A. Emick and Associates already has about me at the time of the change, and any personal information created or received after the change takes effect.

    • Dr. M.A. Emick and Associates encourage you to periodically reread these Terms and Conditions of Use to see if there have been any changes to our policies that may affect you. The date of the most recent update will be clearly posted at the end of this Agreement.

  3. I understand that this Patient Portal is a private internet application provided and maintained by Dr. M.A. Emick and Associates.

  4. I understand that it is a violation of state and federal law to access the private computer network of another, such as this Patient Portal, without prior authorization. Use of another’s user name and password is not permitted and does not amount to authorization by Dr. M.A. Emick and Associates to use this Patient Portal.

  5. I understand that the Portal is NOT to be used for medical emergencies. If I am experiencing a medical emergency, I agree to call 9-1-1 or go to the emergency room.

  6. I understand that the Patient Portal is not continuously monitored, and that my correspondence will not be reviewed in real time. Dr. M.A. Emick and Associates will take every effort to respond to me within two business days of receipt and review of my correspondence.

  7. Only adults (over 18) may access the Patient Portal. Parents of minor children may use this Patient Portal to communicate with Dr. M.A. Emick and Associates on behalf of their minor children.

  8. I understand that the Patient Portal is available as a convenience, and that Dr. M.A. Emick and Associates may terminate access at any time, or for any reason.

  9. I also understand that my participation is entirely voluntary, and I can elect to terminate my access to the Patient Portal at any time.

  10. I understand that the Patient Portal is a means of communication only and serves as a conduit for transmission of data.
    • The Patient Portal does not contain my actual record, nor can my record be accessed via the Patient Portal. My communications with Dr. M.A. Emick and Associates will be added to my actual record within the office.

    • Only the following information about me is stored by the Patient Portal application: my first name, my last name, my user name and my password.

  11. I understand that, upon completion of my treatment, I may no longer have access to the Patient Portal.

  12. I understand that Dr. M.A. Emick and Associates will attempt to provide Patient Portal access without material downtime or interruption, but cannot guarantee access at all times.

  13. I agree that I have no right to copy, modify, change, store, license, assign or exploit the Patient Portal software or content in any manner.

  14. I understand that Dr. M.A. Emick and Associates has security measures in place to help protect against the loss, misuse, or alteration of information transmitted electronically. These measures include encryption of data using the Secure Socket Layer (SSL) system, and using a secured messaging application. However, despite these measures, the absolute confidentiality of any communication transmitted via the internet cannot be guaranteed.

  15. When I registered, I was able to define my own unique user name and password in order to access the Patient Portal.

    • I understand that I am solely responsible for any activities related to my user name and password.

    • I agree not to share my user name and password with anyone else.

    • I agree that, if I believe someone else has gained access to my password, I will request that my password be reset by selecting the Lost your password? option on the Login page.

  16. I understand that I am solely responsible for choosing an appropriate device and environment from which to log in to the Patient Portal.

  17. Dr. M.A. Emick and Associates cannot assure the security of data intercepted or misused by other devices outside its own network. I understand that I am solely responsible for keeping my computer or other device secure, including updating it and its applications with required upgrades, updates and security patches available from third parties.

  18. I agree to always log out of the Patient Portal when I am finished using it.

  19. I understand that Dr. M.A. Emick and Associates may discontinue use of the Patient Portal at any time, for any reason, and without notice.
